Assessment

Referral Process

Patients can be referred to the clinic by their family physician or other medical specialists. Once we receive the completed MMSE (a short memory test that the physician performs with the patient) and the Booking Information Form, we will book the appointment directly with the patient or patient’s next of kin/contact person indicated on the booking form. Currently, our waitlist is about six months long. When the appointment has been established, we will advise the patient or the caregiver, of the date.

Cancellation

If you are unable to keep your scheduled appointment, please inform us at least three days in advance at 604.822.7031.

Confirmation

Please call our office to confirm your appointment two days prior to the day you are to be seen at the clinic at 604.822.7031.


Clinic assessment

Please report to the Admitting Department of the Koerner Pavilion (4 story concrete building with blue roof). The registration clerk will need to know the name of the clinic you will be attending (show them the package that we mailed to you). A volunteer or clinic staff member will then meet you and escort you to your first appointment.

Preparation

It is essential that someone such as a family member or a friend accompany you to the clinic.

If this is your initial visit to the clinic, please take the time to review the map that we mailed to you. Please also note that the initial visit to the clinic is typically 4-6 hours long, so plan your day accordingly.

In order that all medications (including vitamins, naturopathic and over the counter drugs) can be documented on your chart, please ensure that these are brought to the clinic at the time of the assessment.
